What Documents are Required for Golfer's Insurance?

General Requirements

  • Copy of Insurance Policy
  • Copy of Official Receipt of premium payment
  • Receipts showing that the insured practiced or played golf on the day the accident or claim occured
  • Receipt of ball rent (if accident occured in a driving range)

Specific Requirements

  • Third Party Death or Bodily Injury claim
    • Police Report of accident or affidavit stating the particulars of the accident
    • Autopsy report if applicable
    • Death Certificate if applicable
    • Receipts or Invoices of funeral or burial expenses
    • Medical Records and/or medical certificate
    • Hospital statement of accounts
    • Receipts of medical expenses
    • Marriage Contract/Birth Certificate of deceased and/or children if applicable
    • Other evidence to establishrelationship between victim and claimant
  • Third Party Property Damage Claim
    • Police Report, Complaint Sheet or affidavit stating the particulars of the accident
    • Evidence of ownership of property/affidavit
    • Photographs of property showing the damaged parts/portions
    • Estimated value of damaged property or estimate of repair
  • Personal Accident (to the insured) Claim
    • Duly accomplished claim form stating the particulars of the accident and the attending physician's evaluation of the injury
    • Police report of accident
    • Medical certificate/ Hospital reords
    • Death Claim
      • Death Certificate
      • Documents establishing the payee
        • Marriage contract if spouse is the beneficiary
        • Birth Certificate of children if children are the beneficiaries
    • Dismemberment or Permanent Disability Claim
      • Amputation chart describing in detail the extent of the injury
    • Temporary Disability Claim
      • Certification from the attending physician and/or the insured can not yet attend to his usual occupation and when he may return for work
    • Medical Claim
      • Receipts of medical expenses
  • Caddy's Medical Expenses Claim
    • Police report of accident or affidavit stating the particulars of the accident
    • Medical certificate or hospital records
    • Receipts of medical expenses
  • Hole-in-One Benefit
    • Club Bar Bills
    • Extra Caddy Fees
    • Original score card duly attested by 2 accompanying players and one caddy as witnessed
    • Certification issued by the golf club manager stating that the insured made a Hole-in-One
  • Loss or Damage Golf Clothing, Clubs and Equipments
    • Police Report/Affidavit or Sworn Statement of Loss stating the date, time and place, where and how the loss occured
    • Detailed inventory of damaged of lost items showing the date of acquisition/ acqusition cost and extent of loss or damage
    • Photographs of equipment showing the damaged portions
    • Receipt of purchase
