What To Do in Case of a Motor Vehicle Accident?

1. Immediately report the accident to proper police authorities.
2. Accomplish a TRAFFIC ACCIDENT SITUATION FORM/ISAP Form No. 2589/96 (the form is attached to your poliy)
3. In case the driver involved in the accident fled and the vehicle was not identified, gather and collect information from the witness/es at the accident scene.
4. Take necessary steps to prevent further damage or losses to vehicle.
5. In case of injured pedestrian/s or occupant/s of the vehicle, evacuate immediately those requiring medical attention.
6. IMMEDIATELY NOTIFY YOUR INSURANCE COMPANY either orally or in writing.
7. Submit the required documents to support your claim.

What Documents are Required for Motor Vehicle Claim?

1. General Requirements

  • Registration Certificate and O.R. of Registration of the insured unit
  • Deed of Sale if applicable
  • Police Report of Accident
  • Duly accomplished Traffic Accident Situation Form
  • Stencil of insured unit's engine and chassis numbers
  • Copy of insurance policy and certificate of cover
  • Copy of Official Receipt of premium payment
  • Trip ticket (for military vehicles)

2. Specific Requirements

  • Own Damage Claim
    • Photographs of insured unit showing the damaged parts/portions
    • Repair Estimates (at least 2)
  • Theft or Loss Claim
    • Complaint Sheet
    • Alarm Sheet (in case of carnap)
    • Certificate of Non-Recovery
  • Third Party Property Damage Claim (Motor Vehicle)
    • Photographs of adverse vehicle showing the damage parts/portions
    • Repair Estimates (at least 2)
    • Certificate of no claim from adverse vehicle's insurer
    • Certificate of Registration and O.R. of Registration of adverse vehicle
    • Driver's License and O.R. DLR of adverse driver
  • Third Party Property Damage Claim (Other Than Motor Vehicle)
    • Evidence of ownership of property/affidavit
    • Photographs of property showing the damaged parts/portions
    • Estimated value of damaged property
  • Third Party Death or Bodily Injury Claim
    • Autopsy report if applicable
    • Death Certificate if applicable
    • Receipts or Invoices of funeral or burial expenses
    • Medical Records and/or medical certificate
    • Hospital statement of accounts/official receipts/invoices
    • Receipts of medical expenses
    • Marriage Contract/Birth Certificate of deceased and/or children if applicable
    • Other evidence to establishrelationship between victim and claimant
