1. Certificate of Registration of Business Name
2. Contractor's License (for contractors)
3. Audited Financial Statement (for the past two years)
4. Income Tax Return (for the past two years)
5. Community Tax Certificate of authorized signing official
6. Latest Income Tax Return of authorized signing official
7. Mayor's Permit
8. Proof of Billing Address

FOR CORPORATIONS

1. Articles of Incorporation and By-Laws (including the latest amendments, if any).
2. Duly notarized Secretary's Certificate certifying that the official signing the bond and the Indemnity Agreement in behalf of the corporation is authorized to do so by its Board of Directors.
